This is a 'worksheet' form for a invalid pension for Pvt Uri McKee of the 14th NY H. Art. Co. He was wounded badly in the arm at Petersburg, VA on December 17, 1863. He was in the Mt. Pleasant General Hospital in Washington, DC from Mar. 28, 1865 until Aug. 1865. He was discharged from Douglas General Hospital in Washington, DC Sep. 6,1865. Even though this form was never completed, Pvt. McKee did qualify for his invalid pension.
